Instructions:  

1.  Find the service you are/will be providing.
2.  Click on the link for the pre-test for that service.
3.  Print the pre-test and complete.  Don't worry--you may not know   
      all the answers!
4.  Watch the training presentation.  (You will need to obtain a              
      password).  Depending on your computer, you can either press   
      the enter key, use the arrows, or use the mouse to scroll through
      the slides.
5.  Click the link on the post-test for the service.
6.  Print the post-test and complete.  The post-test should have           
     more accurate answers.
7.  Submit BOTH the pre-test and the post-test to CSI for your              
     provider file.

CSI is currently seeking contracted providers for respite and/or adult
companion services.  Hours of service are at the discretion of the individual
and family, as well as the treatment team.  

Contract providers are not employees of the agency.

distributed to service coordinators.
